As a project-based employee of an Open Health member, your health insurance is 100% employer paid. You will have options to purchase a dental\/vison\/life package, buy up from an HMO to a PPO plan, and elect dependent coverage \u2013 all at very competitive prices.<\/div>\n<div>With Open Health, your hours are aggregated across all participating member Employers so you don\u2019t have to meet new eligibility requirements with each new Employer. Open Health coverage is through Anthem Blue Cross.<\/div>\n<div>Depending on plan choice and location, enrolled individuals can have access to the largest national network in the country with more than 900,000 providers. Additionally, Anthem members have access to more than 90% of hospitals and 90% of physicians nationwide. Open Health and Anthem Blue Cross provide employer and employee support through various communication methods, including email, a 24\/7 phone center, and online assistance.<\/div>\n<div>To become eligible for Open Heath, you need to contribute 130 hours in a calendar month during your Initial Employment Period. You become eligible for coverage after a two-month waiting period. During the waiting period you will receive Plan enrollment information. Your insurance coverage will begin June 1.<\/p>\n<p>Your Initial Employment Period begins when you enter Open Health and extends to include a full Measurement Period and the following Admin Period. In this example your Initial Employment Period begins March 1 and ends December 31.<\/p>\n<p>If you average at least 30 hours per week during the Measurement Period, you are eligible for coverage during the next six-month Stability Period. <\/p>\n<p>(NOTE: you do not have to be reporting hours into the Plan during the Stability Period to remain insured although you may not become eligible for coverage during the subsequent Stability Period.)<\/p>\n<\/div>\n<\/li>\n<li>\n<h3>Example 2<\/h3>\n<div>\n<p>You enter Open Health on March 1 and report 130 hours in March while working for participating member employers. You become eligible for coverage after a two-month waiting period, and your insurance coverage begins June 1.<\/p>\n<p>You have no hours reported for 9 consecutive weeks (during June and July); therefore, your coverage ends July 31. You report hours in August and therefore regain coverage September 1 and remain insured until such time a Measurement Period can determine coverage for the next Stability Period<\/p>\n<p> In this example, you averaged fewer than 30 hours per week during your first Measurement Period (May through October), so coverage ends December 31.<\/p>\n<\/div>\n<\/li>\n<li>\n<h3>Example 3<\/h3>\n<div>\n<p>You enter Open Health on March 1 and report 130 hours in April while working for participating member employers. You become eligible for coverage after a two-month waiting period, and your insurance coverage begins July 1.
